I usually enjoy 3 lobes techniques instead of 2 lobes technique. Please see the mucosal and muscular anatomy esp. in the bladder neck area. This patient had naturally separated median and lateral lobes Clear operative field was identified.

Just incise the mucosa first and separate the sphincter during the initial time of HoLEP.
